In order to schedule an appointment, we must have a referral from your doctor. While not a legal requirement, our policy is to ensure screenings complement ongoing patient care. Fortunately, getting a referral from your physician is easy.

Just follow these easy steps:

  2. Show this form to your doctor and discuss which scan you feel would be most appropriate for your situation.
  3. Ask your doctor to sign the referral form.
  4. Call us to schedule your early detection screening at (206) 292-7226.
  5. Bring the signed form with you to your appointment.
